Memberships, Personal Training Sessions, and Class fees are not refundable or transferable Sweet Science Fitness LLC staff reserves the right to rescind the right of members not complying with the terms and conditions of the membership. By agreeing to this plan you are giving Sweet Science Fitness LLC permission to ACH withdraw agreement amount from savings, checking or credit card for the duration of 6 months. Account will automatically renew upon completion if 30 day written notice is not provided 25% return charge for any declined credit card processing or returned checks 30 days written notice will need to be provided prior to cancelling electronic funds transfer and or cancelation of services (no exceptions) By agreeing to this waiver you acknowledge that you will not hold Sweet Science Fitness LLC, and staff liable for any physical injury, health- related injury, illness or death resulting from physical training, fitness conditioning, and/or instruction.
